Acting Commissioner Delaney Announces the Publication of the Final Care Coordination Organization/Health Home (CCO/HH) Application

Dear Friends and Colleagues,

In partnership with the Department of Health (DOH), the Office for People With Developmental Disabilities (OPWDD) is pleased to announce the publication of the final Care Coordination Organization/Health Home (CCO/HH) Application to Serve Individuals with Intellectual and/or Developmental Disabilities (I/DD), as well as the response to public comment on the draft Application. These documents are available for review on the DOH website.

DOH and OPWDD would like to sincerely thank all stakeholders who participated in the public comment period for the draft CCO/HH Application. More than 95 individuals or organizations submitted questions or comments on a variety of topics including transition issues for individuals and families, Medicaid Service Coordination (MSC) staff transition and MSC agency transition issues. Stakeholder engagement and feedback is a crucial component of this process and we appreciate the recommendations and comments received.

Based on the comments received, DOH and OPWDD incorporated several changes to the Application. These changes are detailed in the response to public comment and the Final Application. OPWDD will keep stakeholders updated as we continue on with this transition. Additional information regarding this important opportunity to advance services for people with I/DD is available on our website.
